It is that time of year again! We have started looking at our medical benefits for 2021.
Beginning 9/8/2020, employees can login to FormFire at www.formfire.com and begin updating their applications. The Group number is EEC0F86. Above are a couple FormFire Guides. The first is for returning employees, and the other is for first-time users. First-time users will need to create their own username and password. Returning users may reset their password by logging onto the homepage and following the prompts. These guides will help walk everyone through the process of updating their information. Remember you only need to report a maximum of 5 years history and only “major” things (surgery, hospitalization etc), not minor headaches, colds etc.
Please note that ALL benefit eligible employees, including those currently on COBRA, will need to complete the FormFire application even if you are waiving coverage. If you are waiving, there are only a few questions to answer, and it is a quick process. Please be assured your information will remain secure, as the administrator does not have access to view the applications.
Form Fire will need to be completed by September 22nd.
